Patello-Femoral Joint Pain

 

One of the most common complaints with exercise is patello-femoral joint (PFJ) pain.  This is pain at the front of the knee relating to the kneecap.  It is usually aggravate by loading activities such as squatting, stairs, running etc and it can have a large impact on function. Worse still, ongoing long term knee cap pain has been linked with early onset osteoarthritis of this joint, so it is essential to get onto the injury quickly or better still prevent it from happening at all.

 

Several factors have been linked to PFJ pain

  1. poor quads flexibility – the quads (front of thigh muscles) run over the knee cap so if they are tight they can have a big impact on the knee cap. Make sure you stretch the quads well.
  2. poor inner quads strength but overuse of the outer quads – specialised exercises need to be done to correct this. Massage is a great way to release the overactive outer quads
  3. hypermobility (too much movement) of the knee cap – so if you are naturally a more flexible person you need to do more work to look after your joints with specific exercises
  4. altered foot positioning – this causes too much rotation of the lower leg putting stress on the knee region. Orthotics can be of benefit
  5. poor hip muscle strength – this is where a lot of the focus is now with rehab. If your hip muscles are weak, your hip drops out and your knee drops in. These altered forces can have a dramatic affect on the PFJ resulting in pain. You need strength but also control by using exercises focused on how well you control the leg as you land. By correcting the way we move we are not only improving our muscle control but we are programming our brain with a new and improved set of movement patterns and therefore of muscle memory.
